U.S. Passports

The City of Miramar's Office of the City Clerk proudly serves as an authorized acceptance agency for the U.S. Department of State.  We invite you to schedule an appointment for your U.S. passport from Monday to Thursday, between 7:15 a.m. and 5:00 p.m. If you need a passport photo (ONLY), our convenient walk-in service is available Monday to Thursday, from 8:00 a.m. to noon.  

Step 1: Make an Appointment

  • Online 
  • Only ONE appointment is needed per family. 
  • Complete or Print DS-11 in black ink for each applicant. Do NOT sign prior to your appointment. If you make an error, complete a new form! read all the instruction pages of the DS-11.
  • Bring required originals!
  • Appointments are only available Monday through Thursday between 7:15 AM and 5:00 PM. All applicants MUST appear in person, including infants and children.
  • You will be notified via email with an appointment confirmation. You will also get a reminder email 24 hours before your appointment.

Step 2: What to Bring

  • Bring two passport photos. Do NOT wear white or light (pale or beige) colored shirts. For Babies: Bring a white blanket and carrier.

Payment

Two separate forms of payment is required

  • Passport Application Fees: We accept Personal Check or a Money Order payable to the US Department of State
  • Execution Fees & Photos: We Accept Cash, Credit/Debit, or Money Order

Important Reminders & Renewals

  • Passport services are NOT available when the City is closed including holidays.
  • If you are more than 15 minutes late, you will be asked to reschedule your appointment
  • Renewals for adults using a DS-82 application are done by mail only, unless the passport has expired for more than 5 years.

  • Employees are responsible for relating passport requirements for family members they are booking an appointment for.
